Getting Started
Admission Process
From first call to move-in day, our admissions team moves quickly and keeps you informed at every step.
Initial Referral or Inquiry
Contact our team by phone, email, or through the referral form. Referrals accepted from hospitals, detox centers, agencies, and families.
Documentation Review
We review clinical assessment, medication list, discharge summary, and behavioral history to determine eligibility.
Eligibility Confirmation
Decision made within 24 hours. We confirm placement and coordinate any necessary transition logistics.
Home Tour (Optional)
Families and referring providers are welcome to schedule a tour of the home prior to admission.
Intake & Move-In
Orientation, paperwork, and introductions. The resident is assigned a room and an initial support plan is created.
Eligibility Criteria
- Adults 18 years of age or older
- Behavioral health diagnosis requiring residential support
- Medically stable (not requiring skilled nursing)
- Able to participate in a group residential setting
- Not posing imminent danger to self or others
- Transitioning from inpatient, detox, or crisis program
Documents Needed for Referral
- Clinical assessment (within 30 days)
- Current medication list
- Discharge or transfer summary
- Case manager / provider contact
